Pastors and teachers is one office described by two terms. The best way to describe it is to pastor-teacher or teaching pastor. The grammar of the passage makes this clear in Greek. There is only the one definite article in front of the two terms. They are two parts of one title of one office. Think "Pastors who are teachers" or "Teachers who are pastors."
For some inexplicable reason this is the only time in the entire New Testament in English that the Greek word for shepherd is translated pastor. Pastor is nothing but the Latin word for shepherd. The term pastor (which is shepherd) implies feeding, caring, guidance and ruling. The term teacher implies instruction.
